Your Junior Achievement™ Golf Card entitles you to one free round of golf at each of the following courses (unless otherwise specified):

Cards are $35 each or $30 each for 4 or more cards.
Cards expire Dec. 31, 2008.

GUIDELINES AND CONDITIONS OF PLAY

1. Card holders are entitled to use card once at each participating course, unless otherwise specified.
2. Guests may not accompany a card holder to a private course.
3. The Golf Card will be honored through December 31, 2008
4. Advance reservations must be made by the card holder. Card holders must follow individual club specific reservation policies.
5. Rental of a motorized cart by the card holder is required unless otherwise specified. Cart rates are established by the course and not included in the cost of the golf card.
6. Proper apparel must be worn by players on all courses at all times.
7. The Golf Card is non-refundable and may not be duplicated, reproduced or used in conjunction with other programs without the written consent of Junior Achievement™. The Golf Card will not be replaced if lost or stolen.
8. Junior Achievement™ is not responsible for damage to the golf course and/or equipment.
9. All golf courses have the right to close the course for all or part of any given day.
10. Golf cards may not be used in conjunction with tournaments.
11. Soft spikes only.
12. Identify yourself as a card holder when making reservations.
